To determine if you have mental health coverage, the first thing you should do is check with your insurance carrier.

Melissa Glaab does not belong to any insurance panels. This does not necessarily mean your insurance will not cover Melissa and the services she provides.

You must check with your insurance carrier to see if you can choose your own provider and what your reimbursement for services will be. 

You are responsible for full payment at the time of your appointment and will be provided a superbill which you can then submit to your insurance carrier for direct reimbursement.

Check your coverage carefully and find the answers to the following questions:

  • Do I have mental health benefits?

  • Do I have to see someone on my insurance panel or can I see a therapist outside of network?

  • What is my deductible and has it been met?

  • How many sessions per calendar year does my plan cover?

  • How much does my plan cover for an out-of-network provider?

  • What is the coverage amount per therapy session?

  • Is approval required from my primary care physician?

Cost
Cost for a 50-minute session is $160. I do not offer a sliding scale. 

Reduced Fee 
Reduced fees are offered on a limited basis.

Payment 
All major credit cards are accepted for payment.  I also accept FSA and HSA cards via IVY Pay.

Cancellation Policy 
If you do not show up for your scheduled appointment and have not notified me with at least 24 hours in advanced you will be required to pay the full session fee of $160.

 You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

Good Faith Estimate
• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
